Evaluating a patient's risk for nutritional deficits is an integral part of the hospital practice, ensuring patients who may be nutrition or hydration deficient are identified and receive timely intervention. Validated screening tools and clinical evaluation assist in predicting where a patient is most vulnerable and how to provide appropriate nutritional support, while also monitoring outcomes. Implementing a robust nutritional risk assessment protocol can improve recovery, reduce complications, and decrease length of stay. This session will highlight evidence-based nutritional risk assessment practices, the incorporation of nutrition screening into the hospital framework/pathway, and some approaches to interdisciplinary collaboration that allow clinical teams and dietitians a role in evaluating and ensuring patients receive [...] optimal nutrition, thus improving their care outcomes.
